What to Expect
After scheduling your appointment, you will receive an invitation to our secure patient portal where you can complete your registration paperwork online at your convenience.
If you have difficulty accessing the portal or prefer to complete paper forms, please contact our office and we will be happy to email the necessary documents to you.
For your convenience, registration forms are also available for download. Completing your paperwork prior to your appointment helps us make the most of your visit and reduces wait times.
Office Policies
Treatments: By appointment only. Please call our office at 817-416-0970 to schedule an appointment.
Appropriate attire for your visit: Please wear loose fitting clothes, such as athletic clothes, to allow for full range of motion. If your appointment entails an injection-based procedure, please wear clothing allowing access to the affected areas.
Cancelations: Must be made 24 business hours in advance to avoid a cancelation charge.
