Please arrive on time for your appointment. Due to the nature of our procedures, we require 30-45 minutes for each patient. We make every effort to stay on schedule to respect your time and appreciate the same consideration. Last minute cancellations are not only costly to us, but prevent another patient from receiving needed treatment. Please be courteous.

If you are a new patient, please arrive 15-20 minutes early to fill out the new patient information.

New patient paperwork is also available by clicking the button below for access. Please print the packet and complete the information requested. You can also download the packet, save it as a pdf and fill it out on your computer to print.

Please bring your completed and signed paperwork to your first appointment.

Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire. When you come for your appointments, please remember to bring the following:

  • Driver's License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans etc. and any other relevant information
  • List of medications (if any)

Office Policies

Treatments: By appointment only. Please call our office at 817-416-0970 to schedule an appointment.

Fees: We are now accepting most major medical insurances.

Appropriate attire for your visit: Please wear loose fitting clothes, such as athletic clothes, to allow for full range of motion. If your appointment entails an injection-based procedure, please wear clothing allowing access to the affected areas.

Cancellations: Must be made 24 business hours in advance to avoid a cancellation charge.
